LIPOSUCTION AND ULTRASONIC MINILIPOSUCTION
ACCUMULATED FAT CORRECTION
Liposuction is the technique that has given a new path to correcting accumulated fatty tissue. The difference between classic liposuction and ultrasonic liposuction is that the last one is performed using a specific instrument that sends ultrasonic waves that allows fat to be liquefied and removed more easily.
WHO COULD BE A CANDIDATE FOR AN ULTRASONIC LIPOSUCTION?
The best candidates for ultrasonic liposuction are those patients that have an important abdominal volume.
This procedure also causes a bigger skin retraction.
WHAT TYPE OF ANESTHESIA IS USED IN THIS INTERVENTION?
This procedure can be done using general anesthesia, peridural or analgesic-sedation with local anesthesia. It’s generally an ambulatory surgery, allowing the patient to go home a few hours after the procedure.
ARE THERE ANY MARKS LEFT AFTER THE SURGERY?
Ultrasonic liposuction’s scars are minimal and strategically located to be hidden, even under a bikini. The results may be permanent with the proper diet and the proper physical activity.
LIPOSUCTION'S TECHNIQUE
The surgery can be performed using general anesthesia or local anesthesia with sedation depending on the size of the working area. There are 2 ways of performing the surgery: dray or tumescent (infiltrating a special solution), using cannulas of different length and radius size connected to an aspiration system (an engine, a syringe or an ultrasonic mechanism). Through very small incisions, cannulas are introduced into the working areas. That way it is possible to remove the extra fatty tissue. When the intervention is completed, a compressive mesh or bandage is placed to prevent bleeding and to keep the area from moving. The first check up will be 7 days after the surgery. Stitches are usually removed 7 or 10 days after the surgery and the bandage, after a month.
AREAS THAT CAN UNDERGO LIPOSUCTION
Once more we will insist on the fact that liposuction is not the cure for obesity.
Liposuction can be applied to any part of the body where fat is accumulated: double chins, neck, cheeks, arms, thighs, heaps, buttock, knees, ankles, and abdomen. In men, liposuction can be used to reduce breast fat (gynecomastia) and sides.
EXPECTATIONS
Since the incisions performed are minimal and the working areas are kept from moving with the compressive bandage, liposuction is almost painless. For most patients it’s an ambulatory procedure almost immediately. If general anesthesia had been used, the patient will need to stay for 24 hours. It’s very important to wear the compressive bandage for a few weeks to help the skin retract and to reduce the edema.
RESULTS
Best results are gain with healthy and elastic skin, able to retract after surgery. When tissues are flaccid, the surgeon can recommend a surgical a procedure in which a liposuction is first performed followed then by a tummy tuck. The perfect patient for this procedure is the one that has an average weight with fat deposits in located areas. Liposuction only removes fat, not skin irregularities like cellulites. Thousands of successful liposuctions are performed every year Results can be seen after a few months once the edema disappears and the skin retracts. Probable complications are: pigmentation or scar hypertrophy and bruising. Like in every other surgery, risks need to be discussed with the surgeon.

ULTRASONIC MINI LIPOSUCTION
Ultrasonic mini ultrasonic liposuction is a surgical technique that allows body molding to create a more harmonic and balanced figure. During this procedure, fatty tissue is removed through very small incisions made in different parts of the body.
Ultrasonic mini liposuction is done in one session and the procedure consistst in:
- Marking the working area.
- Making the incisions
- Infiltrating skin with local anesthesia and serum
- Very short ultrasound application
- Removing fat.
This technique is perfect for thin persons that have small amounts of fat that caused a little harmonic body shape, so this treatment is also highly recommended for small working areas.
One of the most important benefits is skin retraction, especially in flaccid areas like arms, thighs and abdomen.
This type of surgery has a very low amount of complications when performed by a qualified professional.
Because of its characteristics allows a fast recovery.

LIPOSCULPTURE
The body is made of fat and fat free tissue (muscles and organs), bones, water and other substances. The amount and distribution of these materials give the body its “characteristic” shape at different ages. For example, a child’s body shape if different form an adult’s, and a young adult’s shape is different from an old person’s.
Fat distribution is determined by three factors:
1.- Genetic predisposition
2.- Environmental Effects
3.- Aging.
GENETICAL PREDISPOSITION
A person’s genetical predisposition is totally characteristic, since some localizes abnormalities can only be corrected with surgery. An example would be the fat accumulation in heaps, which an not be corrected by dieting and that it’s also present in thin patients.
ENVIRONMENTAL EFFECTS
Body characteristics caused by the diet and physical activities are manageable aspects of a person’s body shape proportions. In first world countries, obesity has become a very important pathology. Dieting and exercising have given new impulse to body shaping techniques by reducing the amount of fat, skin or both.
AGING
Aging natural effects and genetical predisposition cause localised fat volume changes in different ways:
- Fat redistribution.
- Muscle loss, that most of the times is replaced by fatty tissue and skin elasticity loss
There is a theory that ever since childhood; fatty tissue develops mainly because of the increasing number of adipose cells. At a certain moment, in general near puberty, the amount of cells stabilizes. From there on, the increase in the amount of cells will be replaced with the increase of the size of the cells.
Liposuction is the surgical aspiration of subcutaneous fat. During the last years it has become the most popular procedure.
Even though age is a very important factor to take into account when going through a liposuction, the general condition of the body is even more important. Ideal candidates are the ones that have exercised and diet and still have fat deposits that disrupt their body shape, over weighted patients can benefit from this procedure even though it’s not a method to lose weight. There are no age limits for this procedure.
ANESTHESIA
The type of anesthesia will depend on the surgeon’s evaluation of the patient (this includes, body structure, the amount of fat to be removed, the general conditions of the patient and the type of surgical technique) according to these parameters, anesthesia can be general, local or by intravenous sedation.
PROCEDURE
Liposuction usually takes 1 to 2 hours, but time may change between 30 minutes to several hours, depending on the working area and the amount of fat that is going to be aspirated. The intervention starts when the surgeon performs a very small incision to introduce he cannula. The other ending of the cannula is connected to the suction engine.
For the last few years the new the Tumescent Technique has been the most used technique. In this technique, saline solutions with lidocaine and adrenaline are injected to the subcutaneous fatty tissue. This infiltration before the suction numbs the tissue and contracts the blood vessels, reducing bleeding, trauma, inflammation and pain.
Almost every area in the body can be treated with liposuction (double chin, nape, cheeks, arms, thighs, heaps, knees and buttocks.)
AFTER SURGERY
After the surgery, some of the wounds may drain for a few days. The patient must use an elastic belt for several weeks starting when the surgery is completed.
You should not expect to see immediate results after the surgery. Treated areas are going to be swollen and you may have a burning feeling too. Regular pain killers control the pain and antibiotic will be prescribed to prevent infection.
YOUR NEW IMAGE
Liposuction is definitely a highly effective technique that can give you a new body outline with very few scars. Results may be permanent, depending on your diet and physical activity. Still if you gain weight again, it will be located in specific areas.
In the last years, men and women have Benefit from this fabulous surgical procedure. A thin and healthy body will give you more self esteem and trust when relating to others.

VIBRATORY LIPOSCULPTURE
Vibratory liposculpture has the advantages of liposuction with ultrasound and improves the timing of a classic liposhculpture, without the risks of the ultrasonic method.
Liposuction is fat removing for aesthetics purposes; can be done using local or general anesthesia and epidural. When liposuction is done to remodel body shape is also called liposculpture.
The surgical technique is performed with very thin 3 millimetres cannulas that are connected to an aspirator to allow subcutaneous fat extraction.
Skin quality is very important in liposuction. Skin capacity to retract is particularly interesting because if its condition is optimum will cooperate to a better result.
The surgery lasts between 1 and 3 hours, depending on the area. Recovery may take almost 2 or 3 months until the final result. The scars are minimum and aesthetic outcomes are very satisfactory.
There are several equipments to complement liposuction. Because of this, new names have been implemented, such as ultrasonic liposuction (liposuction helped by ultrasound), vibratory or pneumatic liposuction (liposuction and compressed air), laser lopilysis (uses laser), etc.
The important result is that liposuction has benefit and satisfied many men and women that had gone through this surgery.
Liposuction has improved a lot in the last decade, thanks to the new equipment and surgical knowledge. Now a day is perfectly well known the skin reaction after the procedure as well as which are the body areas whose skin will retract more easily allowing a better result.
Liposuction performed with a vibratory technique has been becoming more and more popular in the last tow years. This technique is also known as vibratory liposculpture.
These new technique has changed completely the way of doing liposuctions because results are much better and the patients recovery is faster.
This procedure has great advantages. In the vibratory technique uses cannulas connected to an equipment. The equipment moves the cannulas very softly facilitating the procedure and reducing the surgical time. This results in a faster patient recovery, reducing bleeding and bruising.
Vibratory Liposculpture has the same advantages of an Ultrasonic Liposuction (without the needs heat and the ultrasound) and improves the speed of classic liposculpture. Since it uses neither heat nor ultrasound, is impossible for it to burn skin or internal organs, unlike Vibratory Liposculpture, in which that risk does exist.
Vibratory Liposculpture is definitely the newest advance in plastic and cosmetic surgery and is changing the way liposuctions are done.

ULTRASONIC LIPOSCULPTURE
WHAT IS ULTRASONIC LIPOSCULPTURE EXACTLY?
It’s similar to other liposuction techniques in which fatty tissue is removed from the body though small incisions to improve body outline. But Ultrasonic Liposculpture first breaks fat into segments using ultrasonic waves. This ultrasonic energy is distributed to all the fatty tissue though the cannula. This cannula is introduced through incisions same way as n traditional liposuction. Ultrasonic waves remove adipose cells without causing any harm to other tissues, such as blood vessels and connective tissue.
WHAT ARE THE ADVANTAGES OF ULTRASONIC LIPOSCULPTURE?
Even though ultrasonic liposculpture is not a substitution for the traditional liposuction techniques, it has grate benefits, like less pain during recovery.
Some plastic surgeons say that they have a better control of body outline than with traditional liposuction and fewer complications during recovery. This allows removing large fat volumes with almost no blood loss and recovery bothering.
Some areas like heaps and abdomen have a better response to ultrasonic liposculpture. But traditional liposuction is still the best method to remove body fat from areas like face, nape, knees and thighs. Besides, ultrasonic liposculpture must always end with a traditional liposuction.
ANESTHESIA
The type of anesthesia used for ultrasonic liposculpture is the same one used in traditional liposuction. It can be performed under general anesthesia, with the patient totally unconscious, also with epidural or local anesthesia combined with sedation. The type of anesthesia must always be discussed with your personal surgeon.
RECOVERY
The time it takes to recover after this surgery depends on the amount of body fat that was removed. The largest the volume of fat removed and the more areas treated, the longer it will take the patient to recover. After the surgery it is very important to wear a compressive mesh for 2 or 3 months to eliminate the space left by the removed fat. The type of mesh will depend on the treated areas.
IS IT POSSIBLE TO GAIN WEIGHT AGAIN?
Definitely. Patients may gain weight after any type of liposuction. Adults have a constant number of adipose cells. If a person gains weight again the remaining ells will increase their size. Good news is that new body fat after liposuction, spreads in a more harmonious way around the new body outline. It would be best if patients could keep their weight after the procedure.

FAT GRAFT
One of the most traditional techniques in both plastic and cosmetic surgery is to use grafts from different body tissues, like bones, skin, fat and cartilage. For more than 100 years, fat has been used in reconstructive surgery as a filling material in many types of defects. Even other specialties like urology or neurosurgery use fat in some procedures.
Fat grafting has been used in plastic surgery for decades. But it was only some years ago that it became a reliable technique.
Barrancas Medical Center doctors started in 1984 to use this technique, and have been improving it ever since.
WHEN IS FAT GRAFT INDICATED?
Fat is a very wide distributed tissue. It’s easy to remove, handle and reattach. Small or big grafts can be placed in almost every part of the body. Since we are using the patient’s own fatty tissue as our graft, there is no risk of rejection or allergic reaction. Whenever a graft is needed no matter in what part of the body, fat grafting can be used. The first condition for fatty tissue to be used as graft is that there should be a proper amount in the correct conditions to be removed. Skinny patients or with overworked muscle tissue are usually not good candidates. These patients have not only very little amount of subcutaneous fatty tissue, but also their skin strongly attached to the subcutaneous tissue, causing its removal to be very difficult and painful.
Fat grafting can be done also combined to other surgical procedures such as Blepharoplasty, facelift, liposculpture. The best candidates for this type of intervention are those with small or big morphological or aesthetical alterations that seek a better image. This intervention can improve your looks and self-esteem but not necessarily improve your relationships with others or achieve your ideal of beauty.
Fat grafting can be used for gluteus or calf enlargement and filling birth or acquired depressions. It’s the perfect complement for liposculpture to get a real remake and improver of your silhouette.
Now, fat grafting has started to be used in breast augmentation in specially selected patients since it does not apply to every candidate.
